Reports |
| The
State
Reports menu offers reports created for the State of
Washington. It includes patient specific reports and the
state required doses administered report. |

Immunization
Detail Record Report
This report includes dose-specific information
such as manufacturer, lot numbers, injection site, clinic
site, vaccinator, and VIS date. Many providers use this
to replace other handwritten records. |

Immunization Forecast
This report includes the date the last dose was administered
and the immunization status on the day of a clinic visit. |

School
Certificate
Also know as a CIS (Certificate of Immunization Status
form), this report can be printed directly from CHILD Profile,
and is pre-populated with the child's name, date of birth,
and vaccine history. This form is
required for school and childcare entry. |

Vaccine
Administered Report
The Vaccine Administered Report lists the number of
vaccines given broken down by vaccine, dose number and age
group. It meets the state's monthly reporting requirement
for publicly supplied vaccine. This can replace the hand-tallied
"hatch-mark" system that many clinics use to track
vaccine usage. |
