     STEP 5: Getting Your User Account

  • A welcome letter from the Immunization Registry will be sent to your Program Grantee once we receive the signed Information Sharing Agreement. Your Program Director or designee will be asked to notify the Nurse Consultants and other health services staff within his/her program that access to the registry has been initiated.

  • Usernames and passwords will be created centrally by registry staff. Your program nurse will be your contact point. A signed confidentiality agreement is required to get a user account.
    • The program nurse will gather signed Confidentiality Agreements from each staff person assigned to work with the registry. These forms can be kept on site or in the Administrative Office.
    • Using the web account information worksheet the staff nurse will submit his/her name and user account information along with that of other assigned staff. Address and fax number can be found on the worksheet. It is also possible to email the worksheet to elizabeth.nucci@kingcounty.gov.
    • User names and temporary passwords will be sent back to the program nurse for distribution.
    • Please Note: In some instances, the Program Nurse may assign the Health Services Coordinator responsibility to oversee these activities.

  • Once training has been completed and you have your username and password, go to www.childprofile.wa.gov, click on Login, type in your username and password. Welcome! You are ready to search!
