How can I find out which providers in my community participate in the Registry? How about School Districts?
Go to the List of Participating Providers page (www.childprofile.org/Providers/participants.html). Medical Providers and School Districts are listed in alpha order per County.


Who needs to sign the contract (also known as the Information Sharing Agreement)?
The Program Grantee's authorized signator or his/her official designee must sign the Information Sharing Agreement.

Why do you ask for a Technical lead as a contact at the Program?
We ask for a technical lead solely to have a resource internally that is familiar with your system. The Registry is simple to use and secure but sometimes problems arise on a particular computer that we are not able to help with. In this case we would call your Program technical lead to ask for their assistance.

Who can access the Registry? Is it only for nurses?
The Registry is accessed primarily by healthcare professionals. As health care professionals, program nurses can assign the duty to another staff person like a health assistant or family advocate. There is no limit at this time to the number of assignees per program.

Who needs to sign the confidentiality agreement and where do I send it?

  • Any non-health professional assigned to use the Registry by a program nurse must sign the agreement. Many nurses have also elected to sign the form.
  • It is not necessary to send the signed confidentiality agreement to the Registry. It should be held by the nurse at the program administration office.

How do I get a user account?
There is usually a lead person, often the Program Nurse or Health Services Coordinator, who has submitted the other necessary paperwork to the Registry. Once you have reviewed the confidentiality policies and training, the lead will submit your name to the Registry by fax or email. We will send an email including your user name and temporary password to the program lead.

What do I do if I forget my password?
Call the Help Desk at 1-800-325-5599. Passwords cannot be seen by staff as they are encrypted but the staff can reset your password for you.

How can I help to complete immunization records if I have more data? Can I send the CIS form to my Local Health Department?

We know you may sometimes receive records from families that are more complete than information in the Registry. CIS forms are not considered medical records and currently are not considered a verified source of information. Therefore, it is not recommended to update the Registry from a CIS form. Your local Health Department may be willing to update the record if a family presents a shot record. You may wish to make a copy and forward it on to them.

What do you mean by “parental review?” Can a nurse initial information if she/he gets phone confirmation of vaccine information?

  • RCW 28A.210.100 & 28A .210.110 says it is the parent's responsibility to submit immunization information to the program. In utilizing the CHILD Profile Registry, the program nurse or assignee is facilitating parent report. The parent will be directed to review, amend if needed, sign and submit the CIS form to the school.
  • If a signed CIS is already on file, additional information obtained from the CHILD Profile Immunization Registry may be added to the CIS if the nurse or assigned program staff obtain verification from the parent. This verification can be in-person or on the phone. If by telephone, the nurse or assigned program staff must initial and date that a telephone call (TC) was made.
