The CHILD
Profile Immunization Registry offers you
several tools to manage immunizations, generate reports
and update records. These valuable tools will benefit both
you and your staff and save time in your practice.
Easy-to-Print
Immunization Records. You can quickly
and easily generate immunization history reports. Below
are examples of reports that can be created by the
Immunization Registry system:

- Immunization
Summary Organized by vaccine group, this is
an easy-to-read record perfect for giving to parents who need
a copy of their childs immunization history.
- Patient
Immunization Forecast This report shows the next date
a vaccine dose is due and indicates which immunizations are due
now or past due. Some clinics print this out the day of the visit
and place it on the chart, to help determine which vaccines are
needed that day.
- Immunization
Detail This report includes dose-specific information
such as manufacturer, lot numbers, injection site, clinic site,
etc. Many providers use this to replace other hand-written immunization
records, thus eliminating the number of places where vaccine information
is recorded.
- Certificate
of Immunization Status From This form, required for
school and child care entry, can be printed directly from CHILD
Profile, and is pre-populated with the child's name, date of birth,
and vaccine history.
Below is a step-by-step
look at the process for looking up and entering an immunization
record:
1. Patient
Demographic Information
Patient records can be searched a number of ways, for example by
name, birthdate, or chart number. Demographic information for all
children born in Washington State since July 1998 (and for many
older children and adults) is already in the database and can be
updated as needed. New patient information for children born out-of-state,
or for anyone not already in the system, can also be added.
2. Immunization
by Vaccine Group
CHILD Profile produces child-specific immunization forecasting based
on the advisory Committee on Immunization Practices (ACIP) schedule
and the child's immunization history in the Registry. Immunizations
that are current or past due, as well as the dates the next immunization
will be due can be easily displayed and printed.
3. Patient-specific
immunization recommendations
It's easy to enter a patient's current or past immunizations. Multiple
immunizations can be entered on one screen. Many fields can be pre-populated
using clinic-specific "personal settings. A double-click of
your mouse in the date field inserts today's date, or a default
date that you can select. This makes entering a complete immunization
history quick and easy. A child's history with 20 immunizations
can be entered in less than two minutes.
