CHILD Profile Mailings Sign-Up or Corrections

If your child is under age 6 and you reside in Washington State, you should be receiving the CHILD Profile materials by mail. The mailings are sent every 3 - 6 months, and are free of charge.

If you are not receiving the materials, or need to update your child's information, please complete the form below. The information will be used by CHILD Profile staff to enroll your child or update his/her information. You may also email CHILD Profile at
child-profile.health@kingcounty.gov or call the Family Health Hotline at 1-800-322-2588 (voice); 711 (tty relay) to request additional materials or for more information.

NOTE: Please do not sign up a child older than five years old. CHILD Profile is not able to begin mailings to children older than five years old.
