Information for Parents

CHILD Profile is a unique health program for all young children in Washington State. Our goal is to keep Washington's children as healthy and safe as possible.

What is CHILD Profile?

CHILD Profile is Washington State's Health Promotion and Immunization Registry system giving you important health and safety information you need for your child. CHILD Profile helps parents and providers work together to make good health care decisions for children.

Parents of all children born in Washington State get the materials. Children who move into the state can be added to the system by their health care provider, or parents can opt in directly.

What can CHILD Profile do for you?

Health and safety information you can use

CHILD Profile health promotion sends age-specific information to parents of children from birth to six years in Washington State. These materials contain information about immunizations, well child check-ups, nutrition, growth, development and safety.

Easy access to your child's immunization record

The CHILD Profile Immunization Registry is a computer system in Washington State that helps doctors and nurses keep track of the immunizations your child has received. The Registry is a tool for providers to make sure that children get the right immunizations at the right times. The Washington State Department of Health administers the system.

The CHILD Profile Immunization Registry can help you make sure your child's immunizations are up-to-date. The Registry can also provide you with immunization records through your child's health care provider. The Registry may have the dates of all your child's vaccines that you can use for school, child care, and camp entry. Ask your child's health care provider whether he/she is connected to the Registry.

Obtaining your child's immunization information

You may obtain your own or your children's information from CHILD Profile. However, CHILD Profile does not contain complete immunization records for all individuals. It's a good idea to check with your health care provider first to see if they can provide the needed information.

If you cannot get a complete immunization history from your provider, you may contact CHILD Profile to find out if the Immunization Registry contains any immunizations for you or your child. Call CHILD Profile at 206-205-4141 or 1-800-325-5599. Or you may complete the Authorization to Release form (in Spanish: Autorización para entregar documentos de inmunizaciones ), and mail or fax it to:

If you phone, you will be asked to provide identifying information, such as name, date of birth, and address. At that time, you will be informed whether or not a record exists in CHILD Profile, but no other information will be released over the phone. This is to protect the privacy and confidentiality of your information. Once the signed "Authorization to Release" form is received, CHILD Profile will mail or fax the immunization record to you as soon as possible, usually within 3-5 days.

How does CHILD Profile work?

CHILD Profile receives information from the public portion of the birth certificate from the Center for Health Statistics. Your child's date of birth triggers the CHILD Profile system to send each material approximately 30 days before your child's next recommended well-child visit. You will receive a total of 17 mailings from the time of your child's birth until six years of age.

A team of Health Professionals develop and regularly revise each material. A team of parents reviewers and professionals reviewers also review the materials. Additionally, a Spanish Review Team reviews each Spanish material.

Do you want to receive the health promotion materials? Do you need another copy of one of the materials? Do you want to start getting the materials in Spanish? Or do you want to stop receiving the materials? To get these questions answered, call the Family Health Hotline, a program of WithinReach, at 1-800-322-2588 or 711 (tty relay).

If you do not wish to have your child's immunizations entered into the Registry, talk with your doctor or nurse. If they use the CHILD Profile Immunization Registry, they can take steps to honor your request.
