CHILD Profile
health promotion sends age-specific information to parents of children
from birth to six years in Washington State. These materials contain
information about immunizations, well child check-ups, nutrition,
growth, development and safety.

Easy access
to your child's immunization record
The CHILD Profile
Immunization Registry is a computer system in Washington State that
helps doctors and nurses keep track of the immunizations your child
has received. The Registry is a tool for providers to make sure
that children get the right immunizations at the right times. The
Washington State Department of Health administers the system.
The CHILD Profile
Immunization Registry can help you make sure your child's immunizations
are up-to-date. The Registry can also provide you with immunization
records through your child's health care provider. The Registry
may have the dates of all your child's vaccines that you can use
for school, child care, and camp entry. Ask your child's health
care provider whether he/she is connected to the Registry.
Obtaining
your child's immunization information
You may obtain
your own or your children's information from CHILD Profile. However,
CHILD Profile does not contain complete immunization records for
all individuals. It's a good idea to check with your health care
provider first to see if they can provide the needed information.
If you cannot
get a complete immunization history from your provider, you may
contact CHILD Profile to find out if the Immunization Registry contains
any immunizations for you or your child. Call CHILD Profile at 206-205-4141
or 1-800-325-5599. Or you may complete the Authorization
to Release form (in Spanish: Autorización
para entregar documentos de inmunizaciones ), and mail or fax
it to:

If you phone,
you will be asked to provide identifying information, such as name,
date of birth, and address. At that time, you will be informed whether
or not a record exists in CHILD Profile, but no other information
will be released over the phone. This is to protect the privacy
and confidentiality of your information. Once the signed "Authorization
to Release" form is received, CHILD Profile will mail or fax
the immunization record to you as soon as possible, usually within
3-5 days.
